Currently I am an in-network provider with these insurance groups:
Kaiser – First Choice – Premera – Lifewise – Aetna – United – Healthways
PIP (Auto accident) insurance claims are also accepted.
To determine what acupuncture or other medical benefits you have under your plan, call the number listed on your insurance card. Ask: Is a referral needed for me to get acupuncture? How many visits do I have per year? How much is my co-pay / co-insurance? How much is my deductible has been met this year?
We also can call your insurance company and determine what your acupuncture benefits are. Just take a photo of both sides of your card and email it to me along with your date of birth and the primary cardholder’s name and date of birth (if it is not you).
For insurances not listed I can provide you with a ‘superbill’ you can submit for reimbursement, or bill your insurance company as an out-of-network provider.
$90 – 60-75 minute treatment. Treatment includes bodywork and an herbal consult.
$70 – 60-75 minute treatment for youth (17 & under), seniors, students, and veterans.
$50 – 30 minute herbal consult.
Cost of herbal medicines not included. Depending on the specifics of your prescription, prices will vary. The average price of a herbal prescription can range from $10-40. Chinese medicinal herbs are available in pill, granules or raw form.
A hardship discount is available for all patients. Please ask for details
Cancellations made less than 24 hours in advance will result in a
$40 charge. If the appointment is a no-show, the full cost will be
charged.
Appointments almost always begin on time. If an appointment starts more than 20 minutes late, the treatment is free of charge or you can choose to reschedule. If you are running late, please notify by text, email, or calling as soon as you can.
